Obsessive-compulsive disorder is a chronic and disabling disorder characterized by persistent and repetitive thoughts and behaviors accompanied by anxiety. Given the role of the family in the process of obsessive-compulsive disorder in family members, the aim of this study was to compare the effectiveness of acceptance and commitment therapy and mentalization-based therapy on adaptation and communication patterns in spouses of obsessive-compulsive individuals. The research method was a semi-experimental and two-stage (pre-test, post-test) study in three experimental groups (acceptance and commitment group, mentalization group, control group). The research samples were 45 spouses of individuals with obsessive-compulsive disorder who were placed in three experimental groups of 15 people. The results showed that the scores of communication patterns in the post-test of the mentalization group were significantly lower than those in the acceptance and commitment group (P<0.05). However, there was no significant difference between the adaptation scores in the post-test of the acceptance and commitment group and the mentalization group (P<0.05). Also, the adaptation scores in the post-test of the acceptance and commitment group were significantly lower than those in the control group (P<0.05). Therefore, the compatibility scores of the spouses in the acceptance and commitment group and the mentalization group are not different. However, the effectiveness of the mentalization approach is greater than that of the acceptance and commitment approach in the communication patterns of the spouses.
